<p> जीवन में हम सभी को कई हार्मोनल बदलावों (hormonal changes) से गुजरना पड़ता है, और इसकी शुरुआत बहुत कम उम्र में ही हो जाती है। यौवनावस्था (puberty) जीवन का पहला बड़ा बदलाव है, जो हार्मोन के कारण होता है। यह आमतौर पर 6–8 वर्ष की उम्र से धीरे-धीरे शुरू होता है। इस दौरान शरीर के हार्मोन दोबारा सक्रिय हो जाते हैं और शरीर का विकास (growth) शुरू हो जाता है।</p>



<p>लड़कियों में इस अवधि के दौरान एस्ट्राडियोल हार्मोन (estradiol hormone) का स्तर बढ़ता है। इसके कारण स्तनों का विकास होता है और मासिक धर्म (periods) शुरू हो जाते हैं, जिसे मेनार्चे (menarche) कहा जाता है। आमतौर पर पीरियड्स 12–13 वर्ष की उम्र के आसपास शुरू होते हैं। इसके बाद शरीर गर्भधारण (pregnancy) करने में सक्षम हो जाता है।</p>



<p>लड़कों में टेस्टोस्टेरोन हार्मोन (testosterone hormone) का स्तर बढ़ता है। इसके कारण आवाज भारी हो जाती है, मांसपेशियां (muscles) मजबूत होने लगती हैं, चेहरे और शरीर पर बाल आने लगते हैं, और शरीर में वयस्कों जैसे बदलाव (adult-like changes) दिखाई देने लगते हैं। यह कहना गलत नहीं होगा कि हार्मोन एक बच्चे को वयस्क बनाने में बहुत महत्वपूर्ण भूमिका निभाते हैं।</p>



<p class=" translation-block"><strong>हार्मोनल प्लेयर्स (Hormonal Players):</strong>
शरीर के अंदर एक नियंत्रण प्रणाली होती है, जिसे हाइपोथैलेमिक-पिट्यूटरी-गोनैडल एक्सिस (hypothalamic-pituitary-gonadal [HPG] axis) कहा जाता है। यह प्रणाली हार्मोनों को संकेत देती है कि कब और कितनी मात्रा में हार्मोन का उत्पादन करना है। जब यह प्रणाली सक्रिय होती है, तो शरीर एलएच (LH) और एफएसएच (FSH) नामक हार्मोन रिलीज़ करता है।</p>



<p>महिलाओं में ये हार्मोन ओवरीज़ (ovaries) को एस्ट्रोजन (estrogen) और प्रोजेस्टेरोन (progesterone) बनाने का संकेत देते हैं। ये हार्मोन मासिक धर्म (periods), गर्भावस्था (pregnancy) और संपूर्ण महिला स्वास्थ्य (overall female health) के लिए बहुत महत्वपूर्ण होते हैं।

पुरुषों में यही हार्मोन टेस्टिस (testes) को टेस्टोस्टेरोन (testosterone) बनाने का संकेत देते हैं, जो मांसपेशियों (muscles), दाढ़ी के विकास (beard growth), आवाज में बदलाव (voice changes) और यौन विकास (sexual development) में मदद करता है।

सरल शब्दों में कहें तो, एचपीजी एक्सिस (HPG axis) शरीर की वह “बॉस सिस्टम” है, जो लड़कियों और लड़कों दोनों में यह तय करती है कि कौन-सा हार्मोन कब रिलीज़ होगा।</p>



<h3 class="wp-block-heading">प्रजनन आयु (महिलाओं में 13-50 वर्ष की आयु; पुरुषों में निरंतर)</h3>



<p>आइए प्रजनन वर्षों (reproductive years) को सरल भाषा में समझते हैं। पीरियड्स शुरू होने के बाद महिलाओं में हार्मोन का पैटर्न कई वर्षों तक काफी स्थिर रहता है। एक महिला का मासिक धर्म चक्र (menstrual cycle) आमतौर पर लगभग 28 दिनों का होता है और यह हार्मोनों के संतुलन से संचालित होता है। आपने कुछ महिलाओं में देखा होगा कि यह चक्र बार-बार बदलता रहता है; इसका मुख्य कारण भी हार्मोनल असंतुलन (hormonal imbalance) ही होता है।</p>



<p>अब इस 28-दिवसीय चक्र को समझते हैं और देखते हैं कि इस दौरान हमारे शरीर में क्या होता है। दिन 1 से 14 (Day 1–14) के बीच एस्ट्रोजन (estrogen) धीरे-धीरे बढ़ता है। इस चरण में शरीर ओव्यूलेशन (ovulation) की तैयारी करता है। लगभग बीच में, यानी करीब 14वें दिन, एलएच हार्मोन (LH hormone) अचानक बढ़ जाता है, जिससे ओव्यूलेशन होता है। दिन 15 से 28 (Day 15–28) के बीच प्रोजेस्टेरोन (progesterone) अधिक सक्रिय हो जाता है। यह हार्मोन शरीर को गर्भावस्था (pregnancy) के लिए तैयार करता है। यदि गर्भधारण नहीं होता है, तो एस्ट्रोजन और प्रोजेस्टेरोन दोनों का स्तर गिर जाता है, और इसी कारण पीरियड्स शुरू होते हैं। इसके बाद यह चक्र फिर से दोबारा शुरू हो जाता है।</p>



<p>यह हार्मोनल चक्र आमतौर पर 30–40 वर्षों तक काफी सामान्य रहता है। हां, तनाव (stress), आहार (diet), व्यायाम (exercise), नींद (sleep) और जीवनशैली (lifestyle) के कारण कुछ छोटे-मोटे बदलाव हो सकते हैं। यदि पुरुषों की बात करें, तो वयस्क जीवन (adult life) में टेस्टोस्टेरोन हार्मोन (testosterone hormone) काफी स्थिर रहता है, लेकिन 30 वर्ष की उम्र के बाद यह धीरे-धीरे कम होने लगता है।</p>

<p>इस दौरान ओवरीज़ (ovaries) धीरे-धीरे कमजोर होने लगती हैं और हार्मोन का पैटर्न नियमित नहीं रहता। जो चक्र पहले हर महीने एक जैसा रहता था, वह अब अनिश्चित (unpredictable) हो जाता है। इसे सरल भाषा में ऐसे समझ सकते हैं जैसे जब कार का ईंधन खत्म होने लगता है, तो वह एकदम से बंद नहीं होती, बल्कि पहले झटके देती है; ठीक वैसा ही कुछ यहां होता है। महिलाओं में पीरियड्स का पैटर्न पूरी तरह बदल सकता है। कभी पीरियड्स देर से आते हैं, कभी जल्दी, और कभी एक या दो महीने तक आते ही नहीं हैं। ब्लीडिंग (bleeding) कभी बहुत ज्यादा हो सकती है और कभी बहुत कम।</p>



<p>इस समय एस्ट्रोजन (estrogen) कभी अचानक बहुत ज्यादा बढ़ जाता है और कभी बहुत कम हो जाता है। एफएसएच हार्मोन (FSH hormone) का स्तर बढ़ जाता है, क्योंकि शरीर ओवरीज़ को ज्यादा मेहनत करने का संकेत देने लगता है। प्रोजेस्टेरोन (progesterone) भी सही मात्रा में नहीं बन पाता, जिसके कारण सामान्य पीरियड्स को बनाए रखना संभव नहीं हो पाता। <a href="https://pmc.ncbi.nlm.nih.gov/articles/PMC6226272/" target="_blank" rel="noreferrer noopener">pmc.ncbi.nlm.nih+1</a>​</p>

<p>शरीर का मस्तिष्क भाग (pituitary gland) फिर भी ओवरीज़ को संकेत भेजने की कोशिश करता रहता है। इसी वजह से एफएसएच (FSH) और एलएच (LH) हार्मोन का स्तर काफी बढ़ जाता है, लेकिन ओवरीज़ कोई प्रतिक्रिया नहीं देतीं, क्योंकि उनका कार्य समाप्त हो चुका होता है।</p>



<p class=" translation-block">मेनोपॉज़ के बाद क्या होता है?
मेनोपॉज़ के बाद महिलाएं पोस्ट-मेनोपॉज़ चरण (post-menopause phase) में प्रवेश करती हैं। इस चरण में शरीर में एस्ट्रोजन कम ही बना रहता है, और यह स्थिति जीवन भर बनी रहती है, जो आमतौर पर 30–40 वर्षों तक या उससे भी अधिक हो सकती है।</p>



<p class=" translation-block">इस चरण के कुछ लाभ भी होते हैं:<br>
• पीरियड्स स्थायी रूप से बंद हो जाते हैं।<br>
• स्तन कैंसर (breast cancer) का जोखिम थोड़ा कम हो जाता है।<br>

<p class=" translation-block">हालांकि, कुछ चुनौतियां भी होती हैं:<br>
• शरीर का मेटाबॉलिज़्म (metabolism) धीरे-धीरे बदलता रहता है।<br>
• हृदय संबंधी समस्याओं (heart problems) का जोखिम बढ़ सकता है।<br>

<h3 class="wp-block-heading">एंड्रोपॉज़ (पुरुष रजोनिवृत्ति) - पुरुषों में 40 वर्ष और उससे अधिक आयु में होने वाली रजोनिवृत्ति</h3>



<p>एंड्रोपॉज़ (andropause) एक बहुत लंबी प्रक्रिया है, जो लगभग 50–60 वर्ष की उम्र तक चलती है, और कुछ मामलों में इससे भी अधिक समय तक जारी रह सकती है। यह महिलाओं में होने वाले मेनोपॉज़ (menopause) की तरह अचानक नहीं होता। महिलाओं में मेनोपॉज़ के दौरान हार्मोन अचानक गिर जाते हैं, जबकि पुरुषों में टेस्टोस्टेरोन (testosterone) धीरे-धीरे कम होता है। यह प्रक्रिया अक्सर 20–30 की उम्र से ही शुरू हो जाती है। औसतन:</p>



<p class=" translation-block">• कुल टेस्टोस्टेरोन (total testosterone) हर साल लगभग 1% कम हो जाता है।<br>

<h3 class="wp-block-heading">एड्रेनोपॉज़ (30 वर्ष और उससे अधिक आयु)</h3>
